Plan Terms & Conditions

1. Plan Enrollment & Eligibility

  • Anyone is eligible to join Genesis. You do not need to be related to a current member.
  • Enrollment requires submission of a completed registration form and payment of applicable fees.

2. Membership Fees

  • A one-time annual membership fee is required at enrollment.
  • All monthly or annual payments are due in advance.
  • Services cannot be used until full payment is received.

3. Payment Policy

  • Genesis plans are prepaid. All payments are final.
  • If monthly payments are not received:
    • Your plan will be temporarily paused.
    • You may resume services by paying any outstanding balance.
    • A $25 reactivation fee may apply after 30 days of non-payment.

4. Missed Appointments

  • Cancellations must be made at least 24 hours in advance.
  • A $35–$50 no-show fee may be charged for missed or late-cancelled appointments.
  • Repeated no-shows may result in suspension of services.

5. Refunds

  • No refunds are given for services used or active billing cycles.
  • Partial refunds may be reviewed only within 14 days of plan purchase if no services have been used.

6. Service Availability

  • Patients are typically seen within 3 to 7 business days, and in some cases, same-day.
  • Genesis does not guarantee specific appointment times or providers but ensures access through its partner network.

7. Services Included

  • Services vary by plan and may include:
    • General visits
    • Labs and diagnostics
    • Preventive care
    • Referrals to specialists
  • Genesis does not currently provide direct specialty or surgical services. These will be added in phases. Until then, referrals may be provided as a courtesy.

  • Genesis may update terms, prices, or services with notice.
  • Members will be notified via email of any major changes.

10. Future Services Notice

Genesis currently provides access to general care and referrals only. Specialty services, surgical procedures, and post-rehabilitation care will be offered in future phases. Until then, Genesis may refer patients to trusted providers as a courtesy. Genesis is not responsible for care outcomes or costs incurred with referred services.
